THE GRANT CYCLE
The Living Well Foundation announces its Fall 2018 grant cycle. The online application process can be accessed beginning Monday, August 20, 2018. All applicants will use the online application format available to the right of this message. Once you access the grant software, you will create an account for your process, or enter your email address and password if you have previously applied for a grant.
The priorities for the grant cycle are:
- Promoting Health Lifestyles Through Education, Awareness, and Wellness Services
- Improving Access to Healthcare Through Availability and Affordability
- Addressing Special Healthcare Needs of Infants and Minor Children
All applicants must address at least one of the priorities within their proposed project.
The focus for the grant cycle is:
- All ages
- New or expanded programs or services for medical, dental, or behavioral health
- Special emphases to qualified school, hospital, or health clinic settings
- Inclusion of matching funding, a plan for sustainability, and large impact for the region
All applicants must design their request to address the focus for the grant cycle and how your requested funding will improve the health or well-being of your community’s population.
Interested non-profit agencies, public universities or community colleges, parish non-profit hospitals, parish school boards, and other qualifying non-profit entities located in the 8-parish service area of Caldwell, Franklin, Jackson, Lincoln, Morehouse, Ouachita, Richland, or Union Parishes are qualified to apply for grants.
All applicants are encouraged to look on the website at the “Forms and Downloads” section under the “Grants” menu or see frequently asked questions under “Tips and Pointers”.
Letters of intent will be accepted beginning August 20 through August 31, 2018, with completion of applications through September 30, 2018. All letters of intent will be submitted via the online grant software portal on the website under “Grants”, “Apply for a Grant”, and specifically at the “Online Application” menu.

The Living Well Foundation was created from the sale of Glenwood Regional Medical Center to IASIS Healthcare in 2006. It exists to promote the health and enhance the quality of life of residents of the service area. Grants are awarded from income derived from invested assets which will further the mission in the areas of medical, dental, and behavioral health and general well-being for those served.
The eight parishes that make up the service area of the Living Well Foundation are: Caldwell, Franklin, Jackson, Lincoln, Morehouse, Ouachita, Richland, and Union.
The Foundation awards grants based on the 2008 Community Health Assessment (conducted by Professional Research Consultants, Inc.) and the resulting Community Plan. Upon receipt of the assessment, a group of community advisors assisted the Foundation to develop the Community Plan. Both plans are available on the website. The greatest needs for the service area, as determined through the Community Health Assessment and the Community Plan, are: 1) Promoting Healthy Lifestyles Through Education, Awareness, and Wellness Services; 2) Improving Access to Healthcare Through Availability and Affordability; and, 3) Addressing Special Healthcare Needs of Infants and Minor Children. Each grant announcement will identify specific areas of emphases.
**Note: Proposals not meeting the specified areas of emphases for the grant cycle will not be accepted.
The Foundation considers applications from the following types of organizations in the 8-parish service area:
- A tax-exempt organization described in Section 501(c)(3) of the Internal Revenue Code that is not a private foundation described in Section 509(a) of the Internal Revenue Code;
- An organization described in Section 170(c)(1) of the Internal Revenue Code (governmental entities);
- An organization described in Section 511(a)(2)(B) of the Internal Revenue Code (state colleges and universities).
Typically a grant announcement will be posted in mid-August with applications accepted through September 30th. No grant application is accepted after that end date. Interested parties should visit the website periodically for further information.
Grants are typically awarded to non-profits whose projects address the following:
- Activities related to rendering care to the sick and injured or in the promotion of health;
- Activities designed and conducted to promote the general health of the community; and,
- Activities undertaken in cooperation with public or private institutions or agencies engaged in providing health services to residents of the District.
- The grant would help assure the continued existence or creation of accessible, affordable health care facilities or services in the District or the service area that are responsive to the needs of the community.
- The grant would support the provision of health care to the disadvantaged, the uninsured, and/or the underinsured.
- The grant would support the goal of improved health care or wellness to the service area.
- The project would include an education component within its goals for participant self-sufficiency or self-awareness over time.
- The project would have a plan of sustainability upon conclusion of Foundation funding.
- A list of the applicant’s governing board and officers (form provided above);
- A copy of the IRS Determination Letter of 501(c)(3) status (unless the applicant is a government entity or state college or university);
- A copy of the most recent audited or reviewed financial statements of the applicant, including the audit or review report; the most recent unaudited financial statements of the applicant; and a copy of the applicant’s budget for the current fiscal year (unless the applicant is a government entity or state college or university);
- A copy of the applicant’s most recent Form 990 (unless the applicant is a government entity or state college or university);
- A conflict of interest statement (form provided above);
- Letters of reference; and,
- A resolution from the applicant’s board or an appropriate letter authorizing submission of the application.
In-kind contributions are goods or services provided instead of cash or new monies. They can include donated space or materials, or portions of personnel or staff time. If in-kind contributions are listed as revenue sources in your budget, you must also show the corresponding expenses. In-kind contributions are important in order to show:
- All the ways in which the community is supporting your project, even though not everyone is giving cash;
- The true cost of the project – what you would have to spend without the support; and,
- The match of other funds for the project.
The Foundation’s Grant Review Process consists of the following steps:
- An e-mail acknowledging receipt of the application will be sent to the applicant.
- Staff will review the application for completeness and eligibility requirements.
- An experienced grant evaluator will review the application using the Foundation’s criteria.
- The Programs/Grant Review Committee will review the application and the evaluation by the reviewer and make a recommendation to the Board of Directors.
- The Foundation Board of Directors will review the application, the evaluation by the reviewer, and the recommendations of the Programs/Grant Review Committee. Decisions related to funding are made by the Board. Some applications must be approved by the Hospital Service District #1 Board of Commissioners.
- A notification e-mail will be sent to each applicant informing them of the Board’s decision.
- Approval of each grant is subject to conditions specified by the Board and detailed in the Grant Agreement.
